FBACC applications are available to download for viewing offline or printing. Please select the file you need by right-clicking the appropriate link for the document format you want under the application title below.

The BACB highly recommends that you use our new online certification management portal to complete applications. Please click here to create an account if you have not already done so. Your account will allow you to update your contact information, track continuing education events, renew, recertify and pay fees securely via credit card. In the future, your account may allow you to access information for certificants only.

The BACB will still accept paper applications; however a $50 fee will apply. If you would still like to download the paper application, please click the Download button below.
