What if I uploaded and submitted the incorrect self-report documents at the time I’m completing the application?
- If you need to modify or resubmit the self-report form uploaded while you were creating your application, submit the corrected document to Responding to Requests, “selecting Updates About a Notice of Alleged Violation/Open Case” from the dropdown menu. Be sure to reference the case ID they were provided when they submitted the original self-report. The case ID can be found in the confirmation email you received after submitting the initial self-reporting documentation.
