If you are a BCaBA, BCBA, or BCBA-D, you may submit the following Certificate Replacement Request form for an official hard copy of your certificate. Please do not submit this form if you (a) were certified or recertified within the last 6 weeks, as your certificate will be automatically mailed to the address listed in your BACB account or (b) are requesting an RBT certificate, as RBTs are only sent an email confirmation. RBTs may request a copy of their certification email through the Contact Us Form.
This form is for certificants who:
- need a replacement of their certificate
- have had a name change since their certificate was issued
Please allow at least 6 weeks (or longer if you reside outside of the United States) to receive your certificate. If you wish to verify the status of your certification, you may do so by logging into your BACB account or by visiting the BACB Certificant Registry.
Note: We do not issue PDF versions of certificates.
